Extended Clearance Processing

𝗔𝗧𝗧𝗡: 𝗙𝗶𝗻𝗮𝗹 𝗘𝘅𝗮𝗺𝗶𝗻𝗮𝘁𝗶𝗼𝗻 𝗣𝗲𝗿𝗺𝗶𝘁 𝗳𝗼𝗿 𝘁𝗵𝗲 𝟭𝘀𝘁 𝗦𝗲𝗺𝗲𝘀𝘁𝗲𝗿 𝗔𝗬 𝟮𝟬𝟮𝟭-𝟮𝟬𝟮𝟮 Application for the 1st Semester AY 2021-2022 Test Permit is 𝗲𝘅𝘁𝗲𝗻𝗱𝗲𝗱!

All students from the different colleges who failed to file their applications last November are encouraged to fill out the form including our Student Leaders for the AY 2021-2022.

Before you fill out the form, 𝗽𝗹𝗲𝗮𝘀𝗲 𝘁𝗮𝗸𝗲 𝘆𝗼𝘂𝗿 𝘁𝗶𝗺𝗲 𝘁𝗼 𝗿𝗲𝗮𝗱 𝗰𝗮𝗿𝗲𝗳𝘂𝗹𝗹𝘆 𝘁𝗵𝗲 𝗽𝗿𝗼𝗰𝗲𝘀𝘀 𝗳𝗹𝗼𝘄 𝗮𝗻𝗱 𝗿𝗲𝗺𝗶𝗻𝗱𝗲𝗿𝘀 attached in the form 𝘁𝗼 𝗮𝘃𝗼𝗶𝗱 𝗰𝗼𝗺𝗽𝗹𝗶𝗰𝗮𝘁𝗶𝗼𝗻𝘀 𝗮𝗻𝗱 𝗱𝗲𝗹𝗮𝘆 in the process as you can only fill out the form once.
